  • Welcome to the form for Academic work orders.

    DO NOT SUBMIT ONLINE FORMS FOR EMERGENCIES.

    Our office hours are Monday-Friday, 8:00am-5:00pm. If your request is urgent or an emergency, please contact us directly via phone at (310) 338-7779 during our operational hours.

    Outside of our business hours, including weekends, please contact the Department of Public Safety (DPS) at 310-338-2893 and they will be able to dispatch our after hours technicians to assist.

    Recent update: The Lockshop is now under the Department of Campus Safety Services. Quality Assurance will still create work orders on behalf of the Lockshop.

    However, all Lockshop Access Requests must be submitted via e-mail to quality@lmu.edu

    When requesting access for yourself, please CC your direct supervisor. Please continue to include the 9-digit University ID for any individual who needs access.

  • Cart Reservation

    For location information above, please fill in the location of your office.
  • Cart Reservation Policy
    Facilities Management will only allow 2 carts maximum for no more than 5 consecutive days for mechanical and safety inspection.

    If the 3rd day falls on a weekend, the cart(s) may be returned the following business day.

    Cart(s) must be picked up and returned M-F 8:00am - 5:00pm. FM is not responsible for the delivery, return or pick-up of the VIP carts. The cart(s) must be returned on time. Failure to do so may result in a fee or termination of reservation services. A fee may also be incurred if cart(s) are damaged.

    Please note the following policy reminders regarding cart use on campus:

    • Only drive carts if authorized and trained by LMU Environmental Health and Safety*
    • Do not drive on pedestrian walkways (i.e. Dismount Zone/Palm Walk, Alumni Mall, etc.) except during emergencies or if the cart is specifically required to conduct work in a particular area (in which case, a slow speed shall be maintained). 
    • Keep a safe distance from pedestrians and other vehicles. 
    • Keep hands free (no eating/drinking/phone use). 
    • Always use hand signals for turns and stops. 
    • FM VIP Carts may only be used for transporting passengers and small, lightweight items. Larger items (i.e. A-frames, tents, etc.) may not be transported on these carts to prevent damage. If you anticipate the need to transport larger items, please let FM know so that they can provide you with a flatbed cart instead.  

    *ALL cart drivers must be cart certified through Environmental Health and Safety (310.338.7861). By checking the box below you certify that the driver scheduled to pick up the cart has been certified in the past two years.

  • Paint Request Details

    All requests will be assessed in person by our coordinators. The client does not have to be present, however, can request to be present in the request description box below. Assessments are scheduled bi-weekly and you will be notified when your request has been scheduled.

    Assessments are conducted between the hours of 7:30am - 8:30am. Due to the nature of maintenance work, assessment schedules may change. We appreciate your understanding and patience. 

    Please be aware there will be an increase in lead time during our peak times listed below:

    2 weeks before and after the start of school, 4 weeks prior to commencement, and 2 weeks prior to winter break.

    In addition, please also be aware a minimum of two weeks is required for lead time to cooordinate available labor and materials. This lead time does increase during our peak times. 

    We understand there are circumstances for faster turn around times which we can accomodate by contracting an outside vendor. If necessary, please detail this below.

    To find University ID, please take the following steps:

    University ID number can be found on my.lmu.edu login page by clicking on “Account Lookup” under Log In Button and following steps. (9 digit starts with 9, different number than Employee ID and OneCard Number)

     

  • Furniture/Relocation Services (ACADEMIC):

    For any furniture consultation, reconfiguration requests, or purchasing assistance, please directly contact Alex Contreras at Alejandro.Contreras@lmu.edu or 310-338-1916. (ACADEMIC SPACES ONLY)

    For office furniture repairs, please submit a form below.

    For any classroom furniture i.e. desks, podiums, chairs, AV equipment, computers, please conctact Classroom support at classroom@lmu.edu or 310-338-7777.

    For any furniture disposal requests, please submit a work order with photos and dimensions attached. Please understand that a cost may incur.

  • Vehicle Reservation Policy:

    Vehicles must be picked up and returned M-F 8:00A-5:00P. FM is not responsible for the delivery, return, or pick up. The vehicles/carts must be returned on time. Failure to do so will result in a fee or termination of reservation services. A fee for damages may also be incurred if applicable.

    Rentals are to be booked a minimum of 48 hours in advance. We will do our best to accommodate your needs if a sudden necessity arise, but cannot guarantee results. For further questions, contact FM Quality at 310.338.7779.

    For any vehicle rented out from Facilities Management, the renter is responsible for and agrees to pay a fine should damages or missing vehicles occur during rental period.

    Renter acknowledges that he or she possesses a valid driver’s license and is in good standing during the duration of the active terms of the Booking Contract and rental period.

    Renter acknowledges that he or she is at least 18 years of age and is in suitable good mental and physical health to operate machinery and vehicles.

    Renter shall not allow anyone else drive or operate the vehicle who is not specifically listed on the Booking Agreement under the “Driver Names” section.

    Renter shall return the vehicle/cart in the condition that it was found, or cleaner. Any messes made should be cleared out upon returning the vehicle.

    *This form is an indication you are interested in a cart or vehicle. Reservation will be officiated when you come to the Facilities Management building to sign a Booking Form/Contract.*

    Please note that requests related to disposal of large items such as furniture, equipment or other miscelleneous bulk items may incur a cost dependent on quantity, size and weight. We require you to include this information and photos when submitting these types of requests.
