    • Give us the date and time range for when the incident occurred.
    • If you do not know the exact time the incident started, make your best guess.

  • Supplemental Information

    • Use this form to submit additional information about your police report, such as:
      • Pertinant information you might have left off the original report or forgot to tell us about.
      • Additional stolen property. Please download and fill out the Amended Loss form and attach it here or mail it to the address on the form when completed.
      • New suspect or witness information.
  • Stolen Property

    • Be as accurate and descriptive as possible.
    • We cannot enter items into police databases without serial numbers, so please provide this information whenever possible.
    • If an item is homemade or otherwise does not have a brand or model number, enter "homemade" or "NA."
    • If an item has any unique or identifying marks, such as engraved initials, enter this information in the description field. If you need more room, describe it in the narrative box above.
    • If you are reporting a stolen cell phone, it is YOUR responsibility to notify your carrier. 
    • You MUST provide a value for each item stolen. If you do not know, make your best guess on the value.
