"We" use the words "you" and "your" to mean "Buyer" or "Member". The words "we" and "our" refer to Windows of Wellness, Inc. We agree to sell and you agree to purchase the membership services described herein. You agree to pay us for the membership services according to the payment schedule selected above. Your signature below indicates your agreement to be bound by the terms, conditions, rules and regulations of this Agreement as well as those contained in the document entitled "Policies and Procedures" that has been given to you as part of this Agreement. The undersigned of this Agreement are equally responsible for paying it in full.
Your membership dues are due each month until your membership is terminated in accordance with this agreement as stated below.
Your membership is auto-renewable. Following the initial term, your membership will automatically continue on a month-to-month basis until your membership is cancelled. After 12-months, you may cancel your membership at any time by providing 30 days written notice.
You may cancel your membership during the initial term (first 12-months) only in the event that a physician certifies that you are unable to receive massage services or you relocate outside of a 60 mile radius. All cancellation requests MUST be accompanied by written proof of relocation (e.g. Mortgage or Rental agreement, utility bill, car insurance) or submission of Doctor's note. Upon approval of your written request for membership cancellation during the initial membership term, you will be relieved from making any future membership dues payments.
All cancellations require 30 days written notice and are effective 30 days after the date the final EFT payment has been processed. Payments due prior to the effective date will be charged as scheduled.
You have the entire initial and renewed term of the membership agreement to use all membership massages. You may continue to redeem your membership massages as long as your membership is in effect and is paid current. Upon termination or cancellation of your membership, all unredeemed membership massages will expire 30 days after the final EFT payment has been processed.
You may cancel a massage appointment with no charge up until 24 hours prior to that scheduled appointment time. Same day cancellations will be charged a $25 cancellation fee. If you are on a package or membership agreement and do not call to cancel or if you do not show up for your scheduled appointment, you will forfeit that session of your membership.

Your first payment will be drafted on, or after, the date you submit your membership agreement and subsequent payments will be drafted on the same date each month (for monthly payments) or same date each year (for annual payments).
AuthorizationBy signing below, I authorize Windows of Wellness Massage Therapy, Inc. to charge the account I have specified. Monthly dues and/or renewal fees will be withdrawn on or after the same day each month. I understand that Windows of Wellness may continue to charge my account or cancel my membership in accordance with the terms and conditions of this agreement. Additionally, I authorize Windows of Wellness to charge my credit card on file in lieu of presenting it for any services received at my request.

I acknowledge receiving and reading a complete copy of the Membership Agreement Terms before signing. I understand that the rules and regulations in the "Policies & Procedures" document are incorporated into this agreement and are included in my membership agreement.
