  • In order to fill out this form, please have the following information:

    • A description of the activity that you are requesting approval for.
    • Know what Safety Activity Checkpoint your activity falls under, and have read the guidelines.
    • Have the name and address of the activity/event.
    • Who is hosting the event (i.e.troop, service unit, organization, etc.
    • Date and length of activity/event.
    • Number of girls, registered volunteers, and un-registered volunteers you will have at the event (or your min/max)
    • Check to see if your activity/event requires certified instructors.
    • If your activity/event requires certified instructors, have copies of their certifications to attach to this request.
    • If holding the activity/event at a facility, do they carry liability insurance?
