1. Please fill out the information below for each meet your club plans to host. There are spaces for four meet weekends. If your club plans to host more than four weekends, please create a second form for the remaining events.
2. If your club plans to host more than one race meet AND has more than one race secretary, please use the Information section to indicate the name and contact information of the acting secretary for each event.
3. You MUST upload your meet fliers and COVID-19 information for the request to be accepted. You MUST upload the information for each meet you schedule.
3. Responses for all fields for the first meet section are required, to prevent accidental submissions of the form before a meet request is created. All other responses in this section are optional. However, if the required meet fliers and COVID-19 information are not included for each schedule request the meet requests will not be approved.
