  • Before we collect information to schedule your appointment, please review the requirements for a Consistency Letter and Community Acknowledgement Agreement. If either applies, you must submit a Preliminary Plan Review application before your appointment can be scheduled.

  • Please Note:

    PA Design Professionals must share the following documentation at least 3 business days prior to your scheduled meeting (or upload with this form submission):

    • Site Plan illustrating the SFHA on your development site
      • Plans must be of professional quality and drawn to scale (e.g. 1/4″ = 1′ – 0″) 
      • Minimum sheet size is 18” x 24” 
      • Label: Special Flood Hazard Zones (A, AE, Floodway, X, 0.2%, LiMWA line) from effective Flood Insurance Study (FIS), and when a FIS is not available, from the Flood Insurance Rate Maps (FIRM)     
      • Label - mean high tide/MHHW
      • Base Flood Elevation identified using Flood Insurance Study (FIS,) and if not available Flood Insurance Rate Map (FIRM)
      • Conversion factor table for vertical datum, table to include NGVD 29, NAVD 88, and City Datum – converted for Base Flood Elevation and Design Flood Elevation (generally BFE+ 18”) 
      • Existing topography and proposed topography (must use survey topo to idenitify base flood elevation) - do not just copy and paste from FEMA map
      • Building footprints (including accessory structures, transformer pad, etc.) 
        • Floor plans for lowest level (including basement/crawlspace) as well as any floor at BFE+18” and below, must label use of each interior room/space

  • Please select a preferred date and time. Note: Your appointment date and time are not confirmed until you receive a separate notification from staff. Morning (AM) sessions are held between 9am-12pm and afternoon (PM) sessions are held between 1-4pm.

    The applicant and/or co-applicant must also attend all examiner appointments requested by any property owner or contractor. Only contractors who have "Approved" permit involvement on the permit may request an examiner appointment if they are not the primary applicant/co-applicant.
