  • New Household Information Form

  • We're excited to have you as a part of our ministry at Johns Creek United Methodist Church and  that you'll be joining our church. Church membership is not a destination at JCUMC; it's a journey! The information on this form will help our staff and leaders connect with you and help you plug into ministry.

  • *If you have more than 6 people in your household, please submit this form with the first six household members and then email membership@johnscreekumc.org with the details for additional persons.

  • Members of Your Household Joining the Church

  • On this form, we're asking about people in your household who will become church members of JCUMC by baptism, profession of faith, or transfer. You will list others in your household, such as a spouse who will retain his/her membership at another church, on the final page of this form.

The first person in the household is assumed to be the primary adult contact. The relationship of additional household members will be specified by reference to this person.

  • Churches often mean different things by "membership." When answering the following question, please consider JCUMC's definitions:

    • Professing Member: The person is baptized, has made a profession of faith in Jesus Christ, and has formally committed to be a part of the church's ministry. This includes teenagers or older children who have been confirmed.
Baptized Member: The person has been baptized, but has not made a profession of faith or been confirmed. Young children often fall into this category.

If this person is currently a Professing member of another church, Johns Creek UMC will coordinate with that church to transfer his/her membership to JCUMC. If this person is not a Professing Member of another church, or is not sure about his/her church membership, he/she will join JCUMC by profession or reaffirmation of faith in Jesus Christ. If he/she has never been baptized, baptism will be administered as part of the membership covenant.

Young children may join the church by transfer of their Baptized Membership or, if they have never been baptized, they may be baptized at their parents' discretion. While JCUMC encourages the baptism of children, they may still participate in the life of the church if they are not baptized.
