Informed Consent, Acknowledgements & Liability Waiver
I hereby give my approval for my child’s participation in any and all activities prepared by PDX Fencing during the selected camp. In exchange for the acceptance of said child’s candidacy by PDX Fencing. I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless PDX Fencing. and all its respective officers, agents, and representatives and volunteers from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from selected camp sessions and/or field trips.
In case of injury to said child, I hereby waive all claims against PDX Fencing including all coaches, employees, volunteers and affiliates, all participants, sponsoring agencies, and, if applicable, owners and lessors of premises used to conduct the event. There is a risk of being injured that is inherent in all sports activities, including fencing. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death.
Photographs and video may be taken during camp session activities. I give permission and consent that any such photographs may be used by PDX Fencing for informational and advertising purposes. No photos or videos will be sold for commercial use.
Code of Conduct
Campers represent themselves, their families, clubs, and the sport. They are expected to treat other campers, staff, and volunteers with respect and courtesy. No swearing, bullying or disrepect to any fencer, staff, or volunteer will be tolerated. Campers will follow all federal, state and local laws. Violations of these rules may result in a camper being sent home. PDX Fencing staff will make that decision; no refunds of fees will be given for campers sent home due to misbehavior.
