    Our organization, THE ELITE ONES ORGANIZATION, INC. presently functions on a shoe string budget and encourages the participation of volunteers who supports our Mission. Without volunteers, our program would not be possible. Whatever capacity you decide to volunteer, know that you are making a huge difference in a child, or family life. The information on this form is "confidential" and will help us to find the most satisfying and appropriate volunteer opportunity for you.

  • During the course of T.E.O.O. experience there will be occasions when I may be photographed/and or videotaped by staff, sponsors, corporate affiliates, and media.
    T.E.O.O. requests your permission for your participation. By signing below, you may choose to grant or deny T.E.O.O. permission to use photographs or videotape of yourself alone, or in groups, on-line, or in brochures or fundraisers for our use in public undertaking relating to support of T.E.O.O. programs.
    By granting permission, you hereby release T.E.O.O. and hold T.E.O.O. harmless from any claims, judgments or demands which may arise from the use of the above-referenced photographs and/or videotapes.

I understand that I will be volunteering at my own risk and that T.E.O.O., its employees and affiliates cannot assume any responsibilities for any liability, accident, injury, or health problems which may arise from any volunteer work I perform for T.E.O.O.
    I agree that all the work I do is on a volunteer basis and I am not eligible to receive any monetary payments or rewards. STIPEND WOULD BE PROVIDED FOR MEALS AND TRAVELING ALLOWANCES.
