Fill out the information for multiple vehicles whenever they are consigned to the same company or individual. If not please fill out a new letter of intent.

Required documents needed: Copy of title (front & back), re-assigned to shipper with address in USA & copy of Bill of Sales.

I, the customer, hereby authorize American Forwarding & Logistics LLC (AFL) & their employees:
· To act as my forwarding agent for export control and customs purposes· To prepare, sign, declare or swear to any Shipper’s Export Declaration required by law or regulations in connection with the exportation of any commodity shipped by me, the customer· To do all things necessary to ensure compliance with all requirements pursuant to section 19CFR192 of the U.S. Customs regulations: including the use of American Forwarding’s USA Address for shipments, of foreign buyers.· To act for and on its behalf as a true and lawful agent and authorized agent for export control, U.S. Census Bureau (Census Bureau) reporting, and U.S. Customs and Border Protection (CBP) purposes. Also, to prepare and transmit any Electronic Export Information (EEI) or other documents or records required to be filed by the Census Bureau, CBP, the Bureau of Industry and Security, or any other U.S. Government agency, and perform any other act that may be required by law or regulation in connection with the exportation or transportation of any goods presented.
This power of attorney is to remain in full force and effect until revocation in writing and received by American Forwarding and Logistics.
