Please provide the following information:
Temporary Shelter: A temporary residence provided for homeless individuals who would otherwise sleep on the street or a temporary residence provided to individuals in emergency situations.
This is also applicable to children who are in temporary residences awaiting permanent placement in foster care.
Example: Women's shelter or homeless shelter
Hotels/Motels: A temporary residence for homeless individuals usually requiring payment or vouchers for lodging and services on a daily, weekly, or monthly basis.
Multi-Family Housing: A temporary residence where a family is sharing the housing with other families due to financial challenges, the loss of housing, or other similar reasons.
Example: Renting a room or more than one family living in a single home due to financial challenges.
Temporarily Unsheltered: A type of residence for homeless individuals that is not meant for human habitation.
Example: Cars, parks, sidewalks, abandoned buildings, campgrounds, bus and train stations, or living on the street.
None of the Above: Select this option if none of the above living situations apply to this student.

Once this form has been submitted a copy will be sent to the school site. The office will contact you to schedule a time to provide the following important documents for verification:
1) Proof of birth date: must be provided before the child may be admitted. One of the following documents is acceptable: birth certificate, recorder sworn transcript, baptismal certificate, physician statement of birth, passport showing age or other valid proof of birth.
2) Proof of Residence: One of the following documents is acceptable:
* Property Tax Payment Receipt * Rental property contract, lease, or payment receipt * Utility service (electric, gas, water) contract, statement, or payment receipts * Voter Registration * Pay Stub * Correspondence from a government agency * Declaration of residency executed by the parent or legal guardian of a student
3) Immunization records must be originals, stamped by the doctor's office. The school will make a copy to keep on file, with originals returned to the parent. Parents must present proof that the following immunizations have been completed.
