Participation and Medical Release Waiver: I hereby voluntarily permit my child to participate in the KLT Match Play Days held at Sandy Springs Tennis Center. I understand and fully accept that there are risks involved in sports, and that accidents and injuries are common and ordinary occurences of sports. I agree that in the event of an emergency injury or illness, the staff member in charge may act on my behalf and at my expense in obtaining medical treament for my child. Every attempt will be made to call the emergency number conrtact before treatment.
