I hereby authorize Illuminate Community Church to verify all information contained in this application with any references, churches or other organizations, and any individuals. I authorize those who are contacted to disclose any and all information pertinent to serving with minors to Illuminate Community Church. I release all such persons or entities from liability that may result or arise from Illuminate Community Church's collections of all such evaluations or information for its consideration of my application. In addition to this application all teens must read and sign our Policies and Procedures and have their parent, guardian or pastor sign a waiver. Only one teen can serve in a room at a time and you must be at least 5 years older than the kiddos you are working with. If you do not currently attend Illuminate's Youth programs you will be required to meet with a staff member.
