  • Welcome to Fayetteville Animal Hospital’s Boarding & Daycare program. We are excited you have chosen us to care for your loved ones while you are away. Please answer the following questions as accurately as possible. Your answers will affect how we place your dog/cat in situations with our other animals and staff. If you have questions, please notate on the form when you arrive with your pet.

  • Emergency Contact Information, whom ever you choose for your emergency contact please make sure they will answer the phone if and when we call. We have had situations when we call given emergency numbers and the contact never returns or answers our calls. PLEASE make sure your chosen emergency person understands their roll in your dog(s)/cat(s) care while you are gone.

  • The following information will be regarding your dog/cat’s individual information. Please fully read each question and answer them to the best of your ability. If you have questions about the question, please ask we are happy to help you. Your answers will not be judged in any way, they will allow us to take care of your pet to our fullest ability and to keep them and our staff safe.

  • The next set of questions will be medical and behavior based. These answers will lead us to create the most relaxing environment we can provide for your dog/cat. None of the questions we are asking will cause your dog/cat to be excluded or given less attention. These questions are to keep your pet, our staff, and other visiting pets safe.

  • Have you noticed any of the following symptoms in the past 30 days.

  • These questions are about your dog interacting with DOGS (Cat owners do not need to fill this section out; our cats only socialize with other cats of the same family).

  • Payment:

    All clients agree to pay all fees for boarding, daycare, veterinary care, and or other services provided on the day of pick up. Prices are subjected to chance and seasonal rates may apply.

  • Cancelation policy:

    WE REQURIE 48 HOUR NOTICE FOR ALL CANCELED RESERVATIONS, FEE FOR CANCELING WITHIN THE 48 HOUR WINDOW WILL BE THE COST OF ½ YOUR PLANNED RESERVATION. IT WILL BE AT THE DISCRETION OF FAYETTEVILLE ANIMAL HOSPITAL TO WAIVE THE CANCELATION FEE. DURING HIGH VOLUME BOARDING TIME PERIODS WE WILL REQUIRE A DEPOSIT AT THE TIME OF YOUR RESERVATION FOR ½ THE AMOUNT OF YOUR STAY. (IF YOU CANCEL WITHIN THE 48 HOUR WINDOW YOU WILL FORFEIT THAT DEPOSIT)

    Health and Vaccinations:

    We require current vaccines for each guest staying with us for daycare or boarding the vaccines we require: Rabies, Distemper Combination (DHLPP or DHPP), Bordetella (also known as Kennel Cough), and Canine Influenza. We also require every guest to be currently taking flea/tick prevention (We will check every guest for fleas and tick upon entry and will provide treatment if fleas or ticks are found. The fee for flea/tick treatment will be given at the owner’s expense) and have a current negative intestinal parasite screening exam within the past 12 months. If your dog/cat has received vaccines, flea/tick prevention, and intestinal parasite screening at another licensed animal hospital, you agree to provide us a copy of these records. If no records are provided from a licensed veterinary hospital, then we will complete missing medical care at your cost and it will be added to your invoice.

    Although we do everything in our power to prevent the transmission of communicable diseases, it is still possible for your beloved pet to become ill even if fully vaccinated. You, as your dog/cat’s parent, understand this is a possibility and agree to not hold Fayetteville Animal Hospital liable and understand if treatment is required you will be responsible for the cost of care.

    Emergency Medical Treatment:

    While unlikely, it is possible that your pet may become ill or injured during their stay at Fayetteville Animal Hospital. Pre-existing conditions could become aggravated by your pets stay with us. If either of these scenarios require medical attention we will reach out to your emergency contact with the phone numbers, you have provided on this contract. Fayetteville Animal Hospital, at its sole discretion, may engage the services of our staff veterinarian or veterinarian on call at SAVES, or our choice and/or administer medicine or give other necessary attention/treatments to your dog(s), and you authorize us to provide any such services at your additional expense. In cases, we believe to be critical, we may take your dog(s) to the veterinarian before trying to contact you. If you refuse medical treatment for your dog(s), Fayetteville Animal Hospital, at its sole discretion, may engage the services of a veterinarian and/or administer medicine to make your dog(s) as comfortable as possible until picked up by your or your emergency contact. You authorize us to provide any such service at your additional cost. If we cannot reach you or your emergency contact, we will make healthcare decisions for your dog(s) based on the recommendations of our staff veterinarians.

    As your pet’s owner, you agree the costs associated with any medical treatment and the cost of any needed transportation for the purposes of such treatment will be added to your final invoice.

     

  • Group Activity:

    Each pet participating in group play must be spayed or neutered by 6 months of age. Owners understand that during your pets stay at Fayetteville Animal Hospital, he or she WILL be in contact with other dogs. Every effort will be made to ensure the safety of our guests. Owners acknowledge, in the unlikely event of your dog(s) is injured by another dog, you release Fayetteville Animal Hospital from any liability for such injury.

    If your dog(s) injures another dog(s), you will be solely responsible, for any injury to the other dog(s) as well as your own dog, and you release Fayetteville Animal Hospital from any liability for such injury.

    Dog Behavior:

    As the owner, you agree if your dog(s) become aggressive, destructive, uncontrollably disruptive, or acts in any manner that puts Fayetteville Animal Hospital employees in danger we will attempt to notify you or your emergency contact to make other arrangements for your dog.

    You understand as the owner dogs which are kenneled together must be from the same household and very accustomed to being around each other. Owners assumes all responsibility for any aggressive behavior, fighting or injury that one dog may inflict upon another while they are together. Fayetteville Animal Hospital reserves the right to separate any dogs who are boarding together at our discretion for the dogs’ own safety. If your dogs are not compatible boarding in the same kennel unit, you will be billed and responsible for the cost of more than one kennel.

    Photos and Video Release:

    Owners agree to allow Fayetteville Animal Hospital and our representatives and employees to use his or her dog’s name and any images or likeness of his or her dog(s) taken while the dog is staying at Fayetteville Animal Hospital, in any form or format, for use, at any time, in media, marketing, advertising, illustration, trade or promotional materials.

    Personal Property:

    While we do encourage pet owners to bring items from home, we ask that you please limit the number of toys to (3). Blankets and beds will also be accepted. Owners agree Fayetteville Animal Hospital and our employees will not be liable or responsible for any lost or damaged personal property belonging either to the owner or the dog. Owners also understands and agrees dog(s) collar may be removed in the kennel and play areas to prevent injury to the dog.

    Abandonment:

    Owners understand and agree if his or her dogs is not picked up within fifteen (15) calendar days after the scheduled pick up date, or the owner refuses to pay agreed upon charges, the dog shall be deemed to be abandoned (O.C.G.A 4-8-3 (2010) 4-8-3). Owner agrees and has read this entire agreement, you have had the opportunity to discuss and ask any questions you may have, and are completely satisfied with our response, and you agree to these terms.
