Purpose and Instructions:
The purpose of this form is to help provide a level of consistency and to set a minimum standard for information provided for building permit applications. Beginning March 01, 2019, this form must be completed in its entirety by the registered design professional of record for each project submitted as part of the design documents of a building permit application. The design professional of record must complete the form in its entirety, and must then attest to the information provided by affixing their professional seal and signing the indicated area of the second page in accordance with either Title 8 - Article 147 Section 7303 or Title 8 - Article 145 Section 7209 of the New York State Education Law. This form is not a comprehensive code analysis, nor does it guarantee compliance, but is merely a tool to assist in the accurate review of a project. Additional information may be necessary or required to document compliance with the New York State Unifotm Fire Prevention and Building Code (2016), New York State 2017 Uniform Code Supplement, and other applicable laws, rules, and regulations. This form is required whenever a project is to bear the dacsimile of a registered design professional as required by either the Title 8 - Article 147 - Section 7303 or Title 8 - Article 145 - Section 7209, both of the New York State Education Law, and is not a single family, two-family, or townhouse. The City of Syracuse is unable to make changes to this document on behalf of the owner, design professional, or other interested party, and errors will require resubmission of the document.
