Onyx's Legacy Animal Rescue Adoption Application
Onyx's Legacy is an all-volunteer, non-profit, animal welfare organization dedicated to the rescue and placement of stray, neglected and abused animals. We are completely funded through fundraising efforts and public donations. All of our pets are cared for by foster homes and receive complete medical care, which includes deworming, vaccinations, spaying/neutering and microchipping prior to their adoption.
This questionnaire is designed to help Onyx's Legacy and yourself determine if a specific dog is the right one for you and your household. All adopting parties are required to complete this form prior to adoption. Onyx's Legacy reserves the right to deny any adoption or foster application based on answers to this questionnaire and/or interviews prior to adoptions!
Our adoption process consists of a thorough application, home inspection, meet and greet with current pets, approved vet checks, and an adoption fee of $150.

We very carefully place our adoptable pets into homes. Our animals come from local animal shelters and have often been neglected, abused, or abandoned, so we do everything possible to ensure they will have the best quality of life.
