I have read, understand, and will comply with the CADTD competition instructions, including the CADTD Indoor Competition Rules and Regulations available on the CADTD website. I am fully responsible for reading and knowing the CADTD rules for the current season. I certify that all of my students meet our school and district eligibility requirements. I agree to attend the 1000 Oaks Dance Challenge with my students and understand that I am responsible for their behavior throughout the contest. I certify that each of my students has his/her own personal or school insurance and I will bring their emergency cards with me. I understand that the Conejo Valley Unified School District and the 1000 Oaks Dance Challenge Competition Director will NOT be responsible for any injuries occurring at CADTD sanctioned competition event, such as 1000 Oaks Dance Challenge on February 15 , 2025.
