  • Boarding and Daycare Policies

  • Medications: If your pet will be receiving medication during his or her stay, it must be in the original veterinary-labeled containers with instructions for administration.                                

     

    Food: It is highly recommended to bring your pet's regular diet to feed during their stay. Please bring food in either a sealed container or in ziploc storage type bags. PLEASE DO NOT BRING FOOD LOOSE IN A TIED GROCERY BAG!. 

  • Vaccine Requirements:

    In order to protect the health of your pet, this facility requires documentation showing that all pets are current on vaccinations.  Pets that are so young that they have not completed their entire series of vaccinations may not yet be protected and, thus, owners accept any risks of infection.

    Vaccines are required as followed:       

     

    Dogs:  Must be current on the DHPP and Rabies vaccine. They also must be current with in the last 6 months on the Bordetella/ Kennel Cough vaccine.    

                                                   

    Cats:  Must be current on the Rabies vaccine.

  • As the owner/agent of the pet(s) listed above, I hereby certify that Is/are in good health and have not been ill with any communicable condition in the last 30 days. I further certify that my pet(s) have not harmed or shown aggressive or threatening behavior towardsany person or any other pet. I have read and understood the following:

  • I understand that the owners, staff, partners, and volunteers, will not be liable, financially or otherwise, for injuries to my pet, me or any property of mine while my pet is participating in services provided by Pressly Animal Hospital. I hereby accept full responsibility and financial liability for any harm or damage whatsoever caused by my pet while in the care and custody of any PAH employee or owner, whether in the facility or in any public or private setting, only as deemed necessary by the owners of PAH.*
  • I understand that there is stress involved for a pet in a daycare or boarding visit, and some,especially senior pets are prone to problems as a result. I authorize the staff and owners of PAH to make any decision they see proper in the event of an injury or emergency with my pet. PAH staff reserves the right to administer care in an emergency, and I agree to pay any resulting veterinarybills. If an emergency arises, PAH will treat my pet and then contact me once my pet is stable. I agree not to hold responsible, financially or otherwise, any PAH staff member or owner for any resulting injury, illness, or death to my pet while in the care or custody of PAH.*
  • I understand and agree that any problems with my pet, behavioral, medical or otherwise will be treated as deemed best by the owners and employees of PAH in their sole discretion, and in what they view as the best interest of the animal. I understand that there are inherent risks of injury, illness or death in an environment with live animals. I further understand that even in the most controlled environments and well-supervised circumstances, there may be daycare incidents and unpredictable behavior that could result in injury to my pets. After considering these factors, I have decided that the risk associated with placing my pet(s) in a group setting with other pets is outweighed by the benefits of socialization and exercise and stimulation derived by my pet(s).*
  • I desire a socialized environment for my pet while attending services provided by PAH while in their care. I understand that while the socialization and play is closely monitored to prevent injury,it is still possible that during normal play my pet may receive minor nicks, and scratches from rough housing with other pets. Any injuries to my pet will be pointed out by staff upon pick-up. Iunderstand that in the event my pet is responsible for an incident with another pet that results in the need for veterinary care, I am financially liable for the resulting veterinary bill.*
  • I understand that if my pet is not picked up on time or by a date specified in a separate agreement, I hereby authorize PAH to take whatever action deemed necessary for the continuing care of my pet. I will pay the cost of any such continuing care and I understand that if I do not pick my pet up after my specified time PAH will proceed according to the guidelines provided by NC Department of Agriculture abandonment of animals by owner procedure for handling. I also acknowledge that I will be fully responsible for all attorney’s fees and associated costs if I abandon my pet.*
  • I understand by allowing my pet to participate in services offered by PAH I hereby agree to allow PAH to take photographs or use images of my pet in print form, on PAH Facebook pages and anything else associated with publication and or promotion by PAH.*
