    You MUST be 18 or older to submit this application.

    Owning a puppy is a life long commitment.  Before long, your fluffy little golden puppy will be a large, shedding, mud-loving dog that likes to chew on your shoes.  They deserve to be loved anyway! This form helps us get to know you & your family, and determine if one of our puppies would be safe & loved forever, regardless of their 'mistakes'!

    🐾 These puppies are our babies, we have hand raised them & love them completely and we want to ensure that they go to a forever home that loves them the same!! Thank you for taking the time to answer these many questions!

    • Puppy's New Family 

    • Please tell us how your puppy will be living.  We know that this is always subject to change, but please let us in on your current situation.

    • We do not include color choice options when selecting a Golden puppy as there is no way to guarantee a specific shade.  A golden's coat may darken as they mature.

    • If you have a preference on an upcoming litter, you may select your option below. This does not guarantee a puppy from a specific litter will be placed in your home.  Your preference will be considered, however we will only place puppies in homes which we feel are the best fit for each individual puppy.

       



    • What to Expect 

    • Completing this form is the 1st step in the process of adopting a Golden Retriever from Lost Forty Kennels.  However, it does not guarantee a puppy as we reserve the right to refuse placement to anyone for any reason.

      We will reach out to you via email within 2 business days of receipt of your application if you are approved & we'll send you the Deposit Agreement for securing your position on our Wait List.
