• In order to perform the eyebrow Microblading procedure in a safe manner, please answer the following questions truthfully.

I have been quoted the cost of today's appointment, which is for toay's appointment only and applies only fi the shape of the tatto is already established.it will be at an additional cost of $150/hour with a minimum tray set up fee of $50.

I understand that if I have any skin treatments, laser hair removal or laser tattoo removal, plastic surgery or OTHER skin altering procedures, Botox, or other cosmetic injectables, it may result in adverse changes to my permanent cosmetics. I acknowledge some of these potential adverse changes may not be correctable.

I acknowledge some of these potential adverse changes may not be correctable. I understand that in order to receive this procedure I must not have used skin exfoliating products such as but not limited to; Retin A, Retinol, Glycolic Acid and similar acids, hair growth stimulating products such as Latisse, Lashboost or Revitalash etc… for at least 60 days. which may adversely alter the results of my cosmetic work.

I understand the use of Sunblock starting a week after the procedure is necessary to help prevent premature fading of the tattoo.

I have not received Botox, micro needling, or any other medical or spa-type treatment to the area(s) to be treated in over 2 weeks.

I consent to the taking of before & after photographs of the said procedure are a condition of the procedure and become the property of Permanent Enhancements to be used as needed. 

AFTER CARE

IN THE NEXT TEN DAYS YOU ARE REQUIRED TO PAY ATTENTION TO THE FOLLOWING:

Keep your eyebrows dry and clean for the next 10 days.

Do not contact with water. A thick crust will appear and all the pigment will fade.

Do not touch the scab in any other case except while cleaning.

For post treatment care, only use recommended liquids to clean the brows (sterile water for the first day only).

If skin is oily or sweaty make sure you clean the skin when necessary (witch hazel if necessary).

Please do not use any other creams except the ones provided or recommended to you in order to prevent possible infections or allergic reactions.

In the first two weeks after the treatment avoid swimming pools, sunbathing, tanning beds (no sun or tanning for 30 days), sauna, beauty treatments, and intense training accompanied by sweating (sport activities), and avoid contact with dirt/dust.

Permanent Enhancements is not liable in case of improper post treatment care.
