At Catman2, we strive to match each adoptable cat with the right adopter. It's important to us that both the cat and the adopter are happy with their match. Adopting a cat is a big life changing decision - you're adopting a new family member who can live 20+ years! We reserve the right to deny any adoption for any reason.

About You
Are there current pets in the home?*
Please tell us about any current pets in your home. Include species, breed, name and age. Are they spayed or neutered? Do they have a record at a veterinary hospital? *
If you have no pets or your pet is recently deceased, please indicate so.
Are you already a client at a veterinary clinic?*
Please list the name and phone number of any veterinary clinics you have taken your pet(s). If you've had vaccines done at a vaccine clinic, please indicate so. What is the owner name that your pets' records are under?*
Please thoroughly answer this question. Failure to do so will result in an incomplete application.
Do you rent or own?*
Please list your landlord/property manager and their contact information and answer the following: what is the pet policy of your rental? Is a pet deposit required and if so, how much is it? Are there any restrictions in the policy? (You will be required to provide a receipt of the paid pet deposit and copy of the pet policy.)*

Ownership Commitment
With proper nutrition and veterinary care, the average house cat's lifespan is 16-22 years. Can you provide for your cat's wellbeing for that lifespan?*
In the event your cat becomes ill or injured, do you promise to provide them with necessary veterinary care?*
If you ever need to move, do you promise to only look for living arrangements that allow cats?*
Do you agree to give us follow-up reports and photos from time to time?*
Do you agree to contact us in the case that you need to re-home this cat for whatever reason?*
