  • I. Client Information

  • II. Personal Fitness Commitment

  • In consideration of my own, personal fitness needs, I hereby agree to enter into a personal training agreement with JMB FITNESS LLC and agree to commit the time and energy necessary to accomplish my goals as written and reviewed by myself and JMB FITNESS LLC. Client will promptly inform trainer of any changes in medical condition or any new injury.

     


    Knowing the material risks and appreciating knowing an reasonable anticipating that other injuries are a possibility. I hereby expressly assume all of the delineated risk of injury, all other possible risks of injury, and even risk of possible death, which could occur by reason of my participation. I AGREE AND UNDERSTAND.

  • sessions that may go unused for the week will carry over to the following month but no further. Any unused sessions after year commitment will be forfeited. Without proper advance notice of services after 12 month commitment, contract will automatic renew for the following year.

  • V. Rescheduling/Holidays

  • Rescheduling of any session requires a minimum 24 hour notice to avoid charges for that session.

    Note: client will be charged for session if less than 24 hours notice of cancellation is given. Semi-private packages must reschedule with given times of Semi-private slot or session will be forfeited.

    Clients arriving late will receive the remaining scheduled session time, unless other arrangements have been previously made with the trainer.

    If Session occurs on holiday (Christmas Eve Dec. 24th, Christmas Day Dec. 25th, New Years Eve Dec. 31st,  New Years Day Jan. 1st, Easter, July 4th, Thanksgiving Nov. 22nd, Labor Day Sept. 3rd etc.). Session will be fortified with in addition coach having a week of vacation thats non negotiable for rescheduling.

     

     

  • VII. Interruption and Cancellation of Service 

  • A. You may also cancel at any time by written notice to the seller if the following circumstances occur. 

    (1) If by any reason of death or permanent disability, the participant is unable to complete training program, he/she shall be relieved of the obligation of making payment other than for services performed prior to death or onset of disability.

    (2) Substantial physical disability certified by a physician, which makes it permanently impossible for the customer to use the seller's services.

    (3) The customer's permanent relocation is over Sixty (60) miles distance from the facility operated by the seller. Proof of new residence is required.

  • VIII. Photo/Video/Website And Social Media Release Consent

  • I, the undersigned, do hereby grant permission to JMB Fitness to post my and/or my child’s story, photo, or other item, to and for JMB Fitness Web site, Twitter, Snapchat, Instagram and Facebook accounts. 

    I hereby release you, your representative, employees, managers, members, officers, parent companies, subsidiaries, and directors, from all claims and demands arising out of or in connection with any use of said “Materials”, including, without limitation, all claims for invasion of privacy, infringement of my right of publicity, defamation and any other personal and/or property rights. 

    I acknowledge and agree that no sums whatsoever will be due to me as a result of the use and/or exploitation of the “Materials” or any rights therein. 

  • IX. Guarantee of Services

  • JMB FITNESS LLC urges all participants to obtain a physical examination from their physician prior to beginning any exercise program. Under certain circumstances, client may require a physician’s approval prior to beginning a training program. I hereby acknowledge that I have reviewed and agree to the above conditions. Any questions that I had concerning these conditions have been answered to my satisfaction.
