Request for Reasonable Accommodations

The New Jersey FFA Association is committed to providing equal access to our events and activities for all people. Please utilize the Request for Reasonable Accommodations to get special assistance for your student(s). Use this form to request a reasonable accommodation or assistance at least 3 weeks before any state-level events: https://form.jotform.com/NJFFA/accommodations-request . A new form will need to be submitted for each event or student for which a reasonable accommodation is being requested. This information will be kept confidential and will be used only to process the request. Our staff will review the request upon receipt and contact the requestor with additional information.

TIPS For Completing Form

In order to ensure a person is added to the form, you MUST click the "Click to Add Student to Form" or "Click to Add Advisor/Chaperone to Form" buttons.

The quanitiy of "Leadership Resources" MUST equal the quantity for "Meals & Other Incidentals." 

ALL students, advisors and chaperones MUST register for LEAD regardless of their length of stay. 

There is no "discount" button due to the inability to include all options and said button.  An official invoice, including Affiliation Discounts, will be provided shortly after this form is submitted. Each chapter will receive up to a $140 discount on Leadership Resources and Blue Affiliation provides an additional discount of up to $140.

Basic Affiliation provides a discount of up to $140 for Leadership Resources and Blue Affiliation provides a discount of up to an additional $140 for Leadership Resources ($280 total).

Before submitting your form, please download the following forms: Student Code of Conduct & Waiver and Advisor Statement of Assurances.  If you forget to retrieve these documents, they are also located on the Ag Education Bulletin Board.  Completed forms should be scanned and uploaded to this form (use the edit submission link to attach forms after you hit submit).
