Authorization for Pet Care Services

I hereby grant permission for Paw Tenders Pet Care Center and its employees to act in my behalf and in my pet's best interest by obtaining veterinary care at my expense if deemed necessary for illness and injury. I will not knowingly bring my pet into the facility with an illness that is contagious, which includes fleas and tick festation.
Paw Tenders Pet Care Center agrees to exercise all due and reasonable care to prevent injury and illness to my pet; however, in the event of injury and illness, Paw Tenders Pet Care Center, the owners, nor employees, will not be held liable for such injury or illness. I agree that there are inherent risks to my pet in any pet facility, and I accept these risks because they are out weighed by the benefits that Paw Tenders Pet Care Center has to offer.
