No guarantees are made with regard to health, behavior, habits or temperament of any animal. I understand I should keep this animal separate from my current pets for up to 10 days to reduce the chance of spreading contagious diseases the animal may be incubating. I also understand my pets at home should be current on all vaccinations before introducing a new family petI agree to provide booster vaccinations, and veterinary care as needed, and to assume all costs of veterinary care incurred for this animal from this day forward.To assume all risks and responsibilities of ownership and indemnify Anthem Pets against any claims for damages including property and personal injury.I agree that the adopted dog or cat will remain an indoor pet.I understand that Anthem Pets reserves the rights to inquire about the animal at any time and if any of these agreements are broken, Anthem Pets rescue reserves the right to reclaim the animal.If at any time I am unable to provide the best possible care for this animal I am adopting, I understand that I am required to contact Anthem Pets. Anthem Pets does not guarantee intake, however if we can re-home your pet we may request a thirty day notice of surrender. We will ask that you keep your pet in your care until a new family is found or a foster becomes available.I agree to have animal checked by a veterinarian within two weeks of adoption if a problem/illness arises.I agree to provide adequate food, water, shelter, exercise, rules, boundaries, limitations and affection to the adopted animal.**If the dog/cat I am adopting today has not yet been spayed or neutered because they are not of the appropriate age or weight at the time of adoption, I agree and understand that I am required to bring the cat/dog to the appointment scheduled by Anthem Pets. I understand that it is my responsibility to make sure that my pet is spayed/neutered within the agreed upon time frame.
