    If you plan on having your rabbi or officiant manually fill out all the info by hand, you may skip out on filling out this form.  The only information I would need from you is the text you plan on using.  If you are unsure about that, not a problem.  I could consult with your rabbi/officiant about that.

  • Signature Lines

  • Each ketubah has at least 2 (according to rabbinial law) required signature lines, and they are for the witnesses (עד - Hebrew spelling for Witness). Although not required, the additional signature lines are optional.

    Each ketubah typically has 5 signature lines at most.

    Also keep in mind that since the ketubah is a groom's obligation to the bride, the signature for the bride may not be necessary.

    It is customary for both the bride and groom to their father's Hebrew names.  If this information is not applicable for the bride, then she may use her mother's Hebrew name. If otherwise, leave the field blank.
