AGREEMENT: This is an agreement between Paramount Sports Complex and the group named above. Please make checks payable to Paramount Sports Complex, Inc. Your group visit may not be the only activity in our facility at the time. Please note that by signing this contract you are liable for damages caused by participants. CANCELLATION POLICY: There is no charge if your group cancels more than two weeks before the date of the visit. If your group cancels within two weeks of the visit you will be charged 50% of the agreed rate. This will be held as a credit to be used at Paramount Sports Complex within one year. If your group cancels within 24 hours of the visit you will be charged in full and this will be held as credit to be used at Paramount Sports Complex within one year. Print name:
