This does not change your address on any official school records until all residency verification paperwork has been turned in to Mrs. Holtyn in the main office or uploaded with this request. Verification paperwork includes the new signed lease agreement or mortgage and two utility bills clearly showing your name and address. If the lease/mortgage is not in your name, you must also submit a notarized verification form that can be picked up in the office. The two utility bills or other approved documents must be in your name. You may also email copies of these documents to Mrs. Holtyn.
By submitting this form, you agree to provide these documents to the main office within 30 days of occupying the new residence.
